Hospital Indemnity Plan

For an individual who is hospitalized, Hospital Indemnity Plan pays a daily benefit from the first day of hospitalization. Benefits can be used for any purpose. Choices in benefit amounts are available.

Acceptance in the plan is automatic. There are no medical requirements.

Additional plan options provide:

• double benefits for a stay in the intensive care unit
• double benefits for a hospital stay for cancer or an accident
• emergency accident benefits
• home, convalescent and skilled nursing facility benefits
